Event & Funding Forms

The forms below are to be used as a resource for any student organization planning a meeting, event, or trip through the university. All forms must be handed in to the Student Activities Office (107 Nazareth) for approval prior to the event.


Please note that all financial transactions (i.e. deposits, petty cash or check requisitions) must be approved by Callie Frieler, the director of Student Activities.


Just a reminder. If your organization wishes to purchase anything through amazon the SALD office must process that request for you. Fiscal will not reimburse you for amazon orders that you do yourself. If you have any questions or concerns direct them to marywoodclubs@maryu.marywood.edu

Student Activities and Leadership Development (SALD) offers over 60 extra-curricular registered clubs and organizations. All clubs and organizations are student run, under the advisement of a faculty or staff member. All registered clubs are open to membership for all Marywood students.
