Medical Requirements for All Incoming Students
All undergraduate and graduate students are expected to submit an Immunization record, showing documentation of 2 MMR vaccines.
*All documents must be submitted by August 1st for the Fall semester.
Medical Requirements for All Students in Campus housing:
- Immunization record showing documentation of 2 MMR vaccines and a Meningitis vaccine (or waiver) as required by PA law
- A physical exam completed within one year of enrollment date.
*Students that do not meet these requirements will be required to live off campus (Approved by the Board of Trustees, 2000).
Medical Requirements for Utilizing Student Health Services:
- Immunization record
- Physical exam completed within one year of enrollment date
- Health History form (completed by student in the Student Health Portal)
- Authorization for Treatment form completed by the student in the Student Health Portal. If the student is under 18 years of age, the form must be signed by parent/legal guardian.
*Required documentation must be submitted to the Student Health Portal no later than August 1st for Fall Enrollment. We no longer accept paper documents. All documents are to be uploaded, by the student, into the Student Health Portal.
Health Insurance
All full-time students are required to have health insurance (Students enrolled in fully online programs are exempt from this requirement). If you have private insurance coverage you MUST fill out the insurance waiver and provide the name of your insurance carrier and the policy number in order to waive out of the pre-billed plan provided by United Health Care.
Students with
All full time new and transfer students entering Marywood University are required to register with the Student Health Portal EHR (Electronic Health Record).
