Medical Requirements for All Incoming Students

All undergraduate and graduate students are expected to submit an Immunization record, showing documentation of 2 MMR vaccines. 

*All documents must be submitted by August 1st for the Fall semester.

Medical Requirements for All Students in Campus housing:

  • Immunization record showing documentation of 2 MMR vaccines and a Meningitis vaccine (or waiver) as required by PA law
  • A physical exam completed within one year of enrollment date.

*Students that do not meet these requirements will be required to live off campus (Approved by the Board of Trustees, 2000).

Medical Requirements for Utilizing Student Health Services:

  • Immunization record
  • Physical exam completed within one year of enrollment date
  • Health History form (completed by student in the Student Health Portal)
  • Authorization for Treatment form completed by the student in the Student Health Portal. If the student is under 18 years of age, the form must be signed by parent/legal guardian.

*Required documentation must be submitted to the Student Health Portal no later than August 1st for Fall Enrollment. We no longer accept paper documents.  All documents are to be uploaded, by the student, into the Student Health Portal. 

Health Insurance

All full-time students are required to have health insurance (Students enrolled in fully online programs are exempt from this requirement). If you have private insurance coverage you MUST fill out the insurance waiver and provide the name of your insurance carrier and the policy number in order to waive out of the pre-billed plan provided by United Health Care.

Students with a serious or chronic health condition are urged to visit Student Health Services to discuss any health concerns. Also, please ask your physician's office to forward information pertaining to your medical problems and treatment to the Student Health Services Center to enable us to maintain continuity of your care

All full time new and transfer students entering Marywood University are required to register with the Student Health Portal EHR (Electronic Health Record).

*Please do not send any papers to the Health Center.

IMPORTANT NOTE: 

All information must be submitted prior to your enrollment at Marywood. Students who fail to provide this information may not utilize Student Health Services, registration for classes may be delayed, and campus housing may be denied.

    Access & Upload Your Health Information

    To access the Marywood Student Health Portal, a student will use their same Marywood You username and password.

    Uploading instructions:

    Step 1: Scan and save all health documents to your computer, or take a picture of the documents with your smart phone. *Make sure all documents have the student's name and ID. Documents will not be accepted if they do not contain identifying information on them.

    Step 2: Once you have logged into the portal, choose the document you are uploading in the "choose document you are uploading" drop down menu

    Step 3: Click "select file" and locate the file on your computer or smart phone

    Step 4: Click the "upload" button. *You may not see the document immediately, but when you attempt another upload, or log back in, it should be visible.

    *Students will be able to view all the documents that have been uploaded.
